Diabetes Prevention Program (DPP)
This one-to-one connection program is for adults with type 1 and type 2 diabetes who feel alone or burdened by their diabetes and/or those who would like assistance improving their self management skills. Clients are paired with a TLC “teammate” who can best relate to their needs.
