2010 Frankie Awards

Frankie Awards

CDC’s annual Division of Diabetes Translation (DDT) Frankie Awards, named in honor of Frank Vinicor, M.D., former director of CDC’s DDT and a founder of NDEP, recognize exemplary promotion and use of NDEP materials and messages in the past calendar year (2009). Awardees were honored during the DDT meeting on April 13, 2010 in Kansas City, Missouri. This year, the Frankies were competitively judged and awarded in five categories: Use of Media to Promote NDEP, Implementation of an NDEP Program through Interpersonal Channels, Promotion of NDEP Resources to Address Disparities, Collaborative Partnership Using NDEP Resources, and the Frank Vinicor Award of Excellence For Exemplary Use or Adaptation of NDEP Resources in a Comprehensive, Multifaceted Campaign to Address Behavior Change.

California Diabetes Program 
Building Awareness of Diabetes Prevention through NDEP’s Paso a Paso Radio Public Service Announcements (PSAs) 
Use of Media to Promote NDEP

The California Diabetes Program leveraged an existing partnership with Radio Bilingüe, a Latino public radio network which broadcasts in the U.S., throughout the U.S.-Mexico border region, and Puerto Rico. During the hour-long Linea Abierta (Open Line) Spanish-language broadcast, the topics of diabetes, how to prevent the disease, and strategies for overcoming healthy eating and physical activity barriers were discussed. The live broadcasts were combined with 58 airings of NDEP’s 30- and 60-second Paso a Paso radio PSAs and promotion of NDEP’s toll-free number. Dean Schillinger, M.D., Chief of the California Diabetes Program, Health Promotion Specialists Javier Carrillo, M.P.H., and Roger Chene, M.P.H., R.D. served as experts for the call-in shows. Additionally, two people with diabetes, as well as the founder of Radio Bilingüe shared their personal experiences with making lifestyle changes to prevent diabetes. The Linea Abierta program is broadcast on 52 stations in 19 states, in addition to Puerto Rico and Mexico. This promotion reached an estimated 120,000 listeners.

Diabetes Education Recognition Program
Implementation of an NDEP Program through Interpersonal Channels

The North Carolina DPCP developed the NC Diabetes Education Recognition Program (DERP) to address inadequate diabetes self-management education throughout the state. The purpose of this program is to increase access to self-management training for people with diabetes, while providing reimbursement to local health departments. The program also addresses elimination of racial and ethnic health disparities, encourages linkages with statewide and community partners, and facilitates evaluation of its efforts. As an American Diabetes Association recognized “umbrella” program, the NC DERP partnered with local health departments to implement multi-sites across the state. They also utilized several NDEP resources in program curriculum and patient education manuals including, Working Together to Manage Diabetes: A Guide for Pharmacists, Podiatrists, Optometrists, and Dental Professionals and Working Together to Manage Diabetes: Diabetes Medications Supplement. Clinicians are trained annually using these resources as well.

Diabetes Telehealth Prevention Program 
Promotion of NDEP Resources to Address Disparities

The primary purpose of the Diabetes Telehealth Prevention Program is to reduce risk factors for pre-diabetes and diabetes among people at risk in three diverse health care facilities in rural areas of Virginia. These areas have a high prevalence of diabetes and diabetes-related risk factors and limited access to diabetes education. A secondary purpose is to test the feasibility and effectiveness of using telehealth technology to deliver the educational component of the program and using a train-the-trainer model to provide on-site group facilitation and support for behavior change. Program content and activities were modeled after the Diabetes Prevention Program and utilized NDEP’s Power to Prevent: A Family Lifestyle Approach to Diabetes Preventioncurriculum as a teaching resource. A year-long curriculum was developed based on group education, individual goal setting for behavior change, and tracking of behavior change. Personal experiences and resources in the area of lifestyle change were also integrated. Diabetes prevention measures include body mass index, waist circumference, blood pressure, and A1C levels. Questionnaires, surveys, and self-tracking of behavior change will be scored and summarized. In addition, outcome measures will be collected and reported later this year.

Virginia Diabetes Council Education and Empowerment Work Group 
Virginia Diabetes School Medical Management Plan and Protocol 
Collaborative Partnership Using NDEP Resources

A few years ago, the Virginia Diabetes Council (VDC) was approached to take the lead in creating a Diabetes Medical Management Plan and Protocol (DMMPP) for all school districts and providers in the state. The Hampton Roads area health care providers and school nurses implemented a pilot DMMPP from 2008 to 2009 and offered to share materials with the VDC to broaden the use of their work. Later, a subcommittee – including the Virginia DPCP coordinator and VDC members – was convened and tasked with developing statewide consensus on a DMMPP and revising the Hampton Roads protocols. Input was gathered from the Hampton Roads group on the use, benefits, challenges, and overall efficacy of their pilot DMMPP. Representatives were identified from across the state to improve the forms and protocols. In addition, the subcommittee utilized NDEP’s Helping the Student with Diabetes Succeed: A Guide for School Personnel for background information, guidance, direction, and to create the protocol piece of the DMMPP. More than 200 parents, health care providers, and school nurses helped develop the DMMPP that was implemented statewide during the 2009-2010 school year. To date, school nurses from 70 of the 135 Virginia school districts have attended trainings on the DMMPP.

Indian Health Center of Santa Clara Valley (California) 
Diabetes Prevention Program 
Frank Vinicor Award of Excellence

The Indian Health Center’s (IHC) Diabetes Prevention Program (DPP) has developed a 17-week multigenerational lifestyle change program focusing on healthy eating and physical activity. IHC’s DPP is grounded in American Indian traditions such as storytelling, drumming, dancing, and ceremonies. The goal is for participants to lose at least 3 percent of their body weight at the conclusion of the program through increased physical activity and healthier eating. The program is taught by a team of culturally competent professionals including a registered dietician, a certified diabetes educator, a marriage and family therapist, a public health nurse, a kinesiologist, and health education specialists. The evidence-based curriculum includes topics on healthy eating, fitness, stress management, and building support for lifestyle changes. In addition, IHC’s DPP adapted NDEP’s Small Steps. Big Rewards. Your GAME PLAN to Prevent Type 2 Diabetes: Information for Patients Food and Activity tracker. The tracker was provided to participants at the end of weekly sessions and reviewed at the start of weekly sessions. The most successful program participants have indicated that the tracker has had the greatest impact on their success.

Lab results and metrics are being collected on all participants. At follow-up, 71 percent of participants were meeting the goal of 150 minutes or more of physical activity per week. At the end of six months, 33 percent met the goal of less than 100 on a Fasting Blood Glucose Test and no longer had pre-diabetes. Data indicate significant decreases in body mass index, significant increases in fruit and vegetable intake, and increased awareness in knowledge, attitudes, beliefs, and motivation toward lifestyle interventions. Success in IHC’s DPP can be attributed largely in part to the integration of NDEP’s Food and Activity tracker.

Use of Media to Promote NDEP:

  • Colorado DPCPDPCP e-Bulletin, Michelle Hansen
    In an effort to streamline outgoing diabetes communications and to create processes to maximize efficiencies, rather than having key messages lost in blast emails, the Colorado DPCP incorporated NDEP messages in an e-bulletin. This e-bulletin provided eye-catching headlines and linked individuals to the full NDEP information and sources. With this new process in place, the program easily communicated the latest NDEP news to more than 800 subscribers around the state on a bi-monthly basis. In addition, the Colorado DPCP’s creation of an e-bulletin template inspired the Colorado Heart Disease and Stroke Prevention program to use a similar format, which helped communicate and provide information statewide.
  • Washington State DPCPThe Washington State Diabetes Connection, Marcelle Thurston, Francisco Arias-Reyes, Jeanne Harmon, and Laura Pennington
    The Diabetes Connection is the web portal for the Washington State Diabetes Network, a member organization of over 500 health professionals in Washington. The portal serves as a clearinghouse of activities, ideas, best practices, links, and resources relating to the Washington State Diabetes Plan. The portal also fosters collaboration and communication within the network and provides useful, credible information to people and organizations working to prevent and control diabetes among Washington residents. The NDEP website and resources are regularly promoted within the portal. When the revamped NDEP site was launched, it was highlighted in a feature article posted to the Connection’s homepage. Additionally, new NDEP resources and campaigns are often promoted in their monthly e-newsletters to network members. Also, when a new member joins their network, they are provided with a link to the NDEP’s News & Notes e-newsletter and are encouraged to sign up.
  • Idaho DPCPSmall Steps, Big Rewards: Prevent Type 2 Diabetes commercial, Mimi Hartman-Cunningham and Nicole Runner
    The Idaho DPCP purchased air time to promote the NDEP’s Small Steps, Big Rewards Prevent Type 2 Diabetes public service announcement (PSA) on television in major media markets. PSAs were aired in Treasure Valley, Magic Valley, and Eastern Idaho in May and June of 2009. The tagline and call to action was, “If you want more information about preventing diabetes and a free pedometer, call 211 the Idaho CareLine.” The informational packets were mailed to callers and included a pedometer with a walking log and other relevant diabetes resources. The CareLine received 170 requests for additional information.

Implementation of an NDEP Program through Interpersonal Channels:

  • Michigan DPCPCaring for Children With Diabetes – Helping the Student With Diabetes Succeed: A Guide for School Personnel, Jean Chickering, Ann Constance, Paula Ackerman, Dan Diepenhorst, and Anne Esdale
    The Upper Peninsula Diabetes Outreach Network (UPDON) developed a program to provide support and education for parents of children with diabetes living across the Upper Peninsula of Michigan. Parents were identified through affiliation and attendance at a children’s diabetes camp that UPDON helped establish. School issues have been a recurring concern for several parents. Therefore, UPDON offers resources to assist parents of youth with diabetes in working with schools. Recently, UPDON scheduled a parent session followed by a family barbeque. A health psychologist was invited to help parents learn how to deal with inappropriate self-care behaviors. More than 60 parents attended, and some parents shared ways that they have been successful in helping their children manage diabetes. As part of this session, NDEP’s Helping the Student with Diabetes Succeed: A Guide for School Personnel was referenced. In addition, parents received copies of the guide’s sample Diabetes Medical Management Plan and Quick Reference Emergency Plan.
  • Florida DPCPA Healthy Start and A Healthy Finish – A Partnership Between the Florida DPCP and the Capital Area Healthy Start Coalition, Catherine Howard
    The Florida DPCP provided Florida’s Healthy Start Program support workers with Road to Health (RTH) toolkits. The initial train-the-trainer session was held in November of 2009. The Florida DPCP continues to work with Healthy Start Coalitions and the state to promote the toolkit and other NDEP materials. Evaluation efforts are in progress and the Florida DPCP is working closely with NDEP on their evaluation plan to capture process and outcome measures. They found that incentivizing RTH trainers by offering free toolkits and materials for the first three people to schedule a local training effectively motivated the trainers.
  • Washington State DPCPThe YMCA Plan Forward: Diabetes Prevention Program, Laura Pennington
    The Washington State DPCP partnered with four local YMCAs to implement the Diabetes Prevention Program (DPP) in community settings. Participants in the 16-week program learned strategies for eating healthier and being physically active, managing stress, and staying motivated. Each participant received Small Steps. Big Rewards. Your GAME PLAN to Prevent Type 2 Diabetes at the beginning of the program, and printed copies of the food and activity tracker for each week.

Promotion of NDEP Resources to Address Disparities:

  • Washington State DPCPUsing NDEP’s Prevention Toolkit to Promote Diabetes Awareness in Worksites, Laura Pennington
    The Washington State DPCP partnered with their Worksite Health and Productivity Coordinator to spread diabetes prevention messages in counties at high risk and employing low wage earners during National Diabetes Awareness Month. The goal was to raise awareness about diabetes risk factors and promote prevention strategies to a broad audience. Folders were developed containing sample NDEP prevention materials for specific populations, tips on how to display the materials, flyers, ADA’s hardcopy risk test, sample NDEP prevention articles, an introduction letter with diabetes statistics in Washington, and a link to the NDEP website. The packets were mailed to businesses, distributed at the Pierce County Worksite Health and Productivity networking group meeting, and packets were provided during the annual Washington State Healthy Worksite Summit. Overall the feedback was positive and the materials were well received, especially since there was a national “monthly” campaign to coincide with the posting of the materials. Numerous employers requested additional packets and information.
  • NY DPCP, A Tale of Two Cities
    The NY DPCP developed a program that provided culturally appropriate materials for Hispanic/Latino and African American audiences. Two communities were selected as representative of these populations in New York State. Materials were field-tested using NDEP messages among the target populations with groups like faith-based organizations. Materials created for African American audiences were developed using NDEP’s The Power to Control Diabetes Is in Your Hands campaign materials. Materials developed for Hispanic/Latino populations were created using NDEP’s Paso a Paso campaign materials.

Collaborative Partnership Using NDEP Resources:

  • NJ DPCP and the South Jersey Family Medical Centers, Inc., Diabetes Outreach and Education System (DOES), Nuris Rodriguez and Carol Mallette 
    The New Jersey Department of Health and Senior Services (NJDHSS) Diabetes Prevention and Control Program (DPCP), in partnership with the Southern Jersey Family Medical Center, Inc. (SJFMC) used NDEP’s ABCs of Diabetes PSAsPower to Prevent billboards, and Managing Diabetes PSAs for the 2009 World Series and PSA/video announcement in over 200 partner supermarkets. The campaign gained the support of the then New Jersey DHSS Commissioner of Health, Heather Howard, by engaging in NDEP PSA taping sessions and appearances in partnership with SJFMC and CBS Radio. The campaign increased community access to SJFMC services for people with diabetes and encouraged the community to learn more about diabetes prevention and complications.
  • Wisconsin DPCPType 2 Diabetes Prevention and Awareness Table Tents, Pam Geis 
    The Wisconsin DPCP created a 9-inch tall, three-sided, full-color table tent with information on what diabetes is, who is at risk for type 2 diabetes, the signs and symptoms of type 2 diabetes, and how to prevent the disease. Corporations displayed the table tents in cafeterias, break rooms, reception areas, bathrooms, etc. They found that table tents are an effective way to communicate public health messages, encourage discussion among employees, and increase website traffic.
  • Kentucky DPCPControl/Manage Your Diabetes, Know Your ABCs, Linda Leber 
    In November 2009 for National Diabetes Awareness Month and World Diabetes Day, the Kentucky Diabetes Prevention and Control Program (KDPCP) partnered with local health departments, local diabetes coalitions, the Kentucky Diabetes Network, KY’s AADE Chapters, the Personnel Cabinet, and the DPH Department of Communications. Partners promoted NDEP’s messages Know Your Diabetes ABCs and Managing your diabetes isn’t easy, but it’s worth it using billboards, health fairs/displays, professional education activities, articles in local newspapers and newsletters, community marquees, theater ads, partner websites, television and radio ads, paycheck stubs, and presentations to public and professional groups. All participants were asked to report on their activities. Total exposure to the November messages was approximately 1,236,872, including about 675,068 in the Appalachian population.
  • Utah DPCPUtah DPCP Gets Magnetic, Darin Larson, The Utah Health Plan Partnership, Utah Medicaid Office, HealthInsight, and Utah Women’s Health Information Network (UWIN)
    The Utah Health Plan Partnership, comprised of eight Utah health plans, recognized that people with diabetes needed an easy way to track and record their test results and decided that erasable refrigerator magnets would be ideal. The Partnership organized a work group to develop the content for the magnets. The work group agreed to emphasize A1C, blood pressure, and cholesterol and adapted NDEP’s Tips to Help You Stay Healthy tip sheet. An electronic version of information on the magnet was placed on the Utah DPCP’s website. In addition, an announcement and link were promoted in health plan newsletters and the Utah Medical Association newsletter, as well as via the Utah DPCP listserv. Magnets were printed in English and Spanish, mailed to over 10,000 health plan members, and provided to diabetes educators and physicians to give to patients with diabetes. Physicians at community health centers, who typically treat uninsured patients, found them to be an especially valuable resource.

Frank Vinicor Award of Excellence for Exemplary Use or Adaptation of NDEP Resources in a Comprehensive, Multifaceted Campaign to Address Behavior Change:

  • South Carolina Division of Diabetes Prevention and ControlSouth Carolina Division of Diabetes Prevention and Control Multimedia Campaign, Rhonda Hill
    The South Carolina Division of Diabetes Prevention and Control has used non-recurring state diabetes dollars to launch a multi-media campaign and is implementing NDEP messages. They are implementing the Get Real, Small Steps. Big Rewards, and Control Your Diabetes. For Life. campaigns. They are promoting these campaigns through voiceover radio spots with heavy rotations on African American and Hispanic/Latino stations. Each of the five NDEP TV commercials have been featured on the “IMARA Woman” TV show and the Get Real andControl Your Diabetes. For Life. television commercials have been in rotation on various major TV stations across the state. NDEP print ads and articles have been published in the Statenewspaper, community weekly newspapers, daily newspapers, Home and Garden magazine (South Carolina only), and IMARA Woman magazine. NDEP posters have also been disseminated to local health departments and community health centers. Evaluation of the campaign, including media tracking, is ongoing.
